The role of central sensitization in refractory pain-related diseases has not yet been clarified. We performed a multicenter case-controlled study including patients with various neurological, psychological, and pain disorders and 5, healthy controls to investigate the impact of central sensitization in these patients. Patients were categorized into 5 groups based on CSI-A scores from subclinical to extreme. CSS-related symptoms were associated with pain severity, pain interference with daily activities, and depressive symptoms in various pain-related diseases. Our findings suggest that CSS may participate in these conditions as common pathophysiology. Central sensitization represents enhanced functions of circuits in nociceptive pathways and an abnormal state of the nociceptive systems, resulting from the remarkable plasticity of the somatosensory nervous system [ 1 ]. Due to alterations in the somatosensory system from high- to low-threshold pain hypersensitivity in this condition, central sensitization pain occurs in the absence of noxious stimuli.

Chronic pain

Previous reviews have reported that manifestations of pain sensitization may play an important role in the pain experienced by people with knee osteoarthritis. However, it is unknown if manifestations of pain sensitization are common features across other painful knee disorders or if sensitization requires targeted intervention. This review aims to synthesize the published research investigating manifestations of pain sensitization in painful knee disorders and to evaluate if the manifestations of pain sensitization change in response to treatment. We searched Medline, Embase, CINAHL, Web of Science, Sportsdiscus, and Cochrane Central for studies that investigated between-group differences knee pain vs pain-free subjects or the effect of treatment on manifestations of pain sensitization. Two reviewers independently assessed studies for inclusion and quality. Available data were synthesized via predetermined levels of evidence, meta-analysis, and metaregression where possible.

Several methods have been developed, including clinical indicators and psychophysical measures.
